YOU are the Center of Your Care!
Bridge Clinic functions as a patient-centered medical home. A patient-centered medical home helps organize your care inside our clinic and with healthcare providers you use outside of our clinic. In a patient-centered medical home, YOU are the center of your healthcare team. You and the members of your team will design a wellness plan that sets goals you want to achieve. Your Bridge Clinic team will provide you with the care and resources you need to meet those goals.
How this Benefits YOU:
- We’ll make it easy for you to get the care you need. We offer same-day and next-day appointments, evening appointments, and after-hours telephone access to healthcare professionals.
- We’ll be able to help you with your medical, dental, and mental health care needs because we offer all of these services right in our main clinic.
- As a Federally Qualified Health Center (FQHC), we can help you even if you have financial concerns. We serve all patients, regardless of their insurance coverage. Our Patient Financial Advocates (PFAs) will help you find payment and insurance options.
- You’ll have access to your own medical chart 24 hours a day, 7 days a week through PrimePATIENT.
- We’ll organize your care with other healthcare providers. This makes sure you get the care and communication you need. These providers will become part of your healthcare team.
- You’ll have a care plan made for you and your ongoing health conditions, such as diabetes, high blood pressure, asthma, or weigh concerns.
YOUR Responsibilities to the Team
YOU are the most important team member when it comes to your health. Please give your care team all the information they need to help you meet your goals. This includes information about medicines you take, your and your family’s health history, special needs you have, and where else you go for care. We will need to get records for any care you receive outside of the clinic. Please be sure to transfer or copy outside health records to the clinic.
